On your first visit, please arrive at least 15 minutes early to give us your personal information. Let us know if you need to take medication before dental treatment. Also, please bring along any medications you currently take so we can record types and dosages. The more we know, the better we can care for you.

We will also need information about your insurance coverage. Please remember to tell us if there have been any changes in your health or insurance information between visits so we can update your records.
